I just recently installed a new oak floor over the floor leveling compound used throughout my condo. The floor lever was very heavily damaged in portion from years of abuse. I filled any cracks, then nailed 3/4" CDX plywood through the floor leveler and into the plywood subfloor using 10D nails. This anchored the floor leveler and gave a good base for the red oak strip flooring I installed. One could do the same thing for prefinished flooring. You just make to make sure to butt the plywood base very tightly to all the surrounding pieces, making the seams good enough to completely support the new surface flooring material. As an aside, I am planning on installing a home theater system in a few months. To allow me to run the wire for the rear surround speakers, I left a 1/2" gap between the plywood substrate and ran the speaker wire under the new wood floor. This gives me several advantages: the rear speakers are now protected from damage and being a trip hazard; if the wires ever need to be replaced, the old wires can be used to pull the new wiring into place so they too will be protected against damage.

Cut along the length of the joists but only half way on the width so you have something to screw in your new subfloor.
